To visit with one of our experienced pain specialists, simply give us a call, schedule online, or submit the "Contact Us" form below. Before your first visit, you will receive an email and text with paperless intake forms however you can print a hard copy (below) if you'd prefer. We look forward to helping you find relief and improve your quality of life.
Getting to the Clinic
Clinic Location and Parking
Our clinic is located conveniently in the Galleria (5373 W Alabama St #204, Houston, TX 77056).

To reach the clinic, take I-610 to the Westheimer Road exit. Head east on Westheimer Road, then turn left onto Yorktown Street. You will then take a left onto W Alabama St (headed east) and then take the first right onto S Rice Ave. You will then turn right onto the ramp to the second floor parking structure (see video below). You will then be able to walk into clinic.

Parking is free on the second floor of the building. You can view video instructions on accessing the parking space and clinic below.
Insurance Information
Accepted Insurance Plans
We accept all major Medicare, Medicare advantage, and commercial insurance plans to ensure that our patients have access to the care they need. In addition, we are happy to assess personal injury and worker's compensation patients.
