If you'd like to refer to our practice, please send the patient's face sheet and last clinic note with any relevant imaging via fax (713-353-3698). You may alternatively message our clinic directly via UpDox.

We accept all major insurance providers as well as personal injury and workers' compensation patients. In addition, we do offer anesthesiology services (including office sedation, OR anesthesia, or preoperative clearance).
